CHAOSRXOptics runs employer-paid prescription safety eyewear programs with no required kiosk or on-site visit. Employees order from a phone or computer on a portal built for the employer, virtual sizing helps employees compare supported frames, and the employer chooses between a monthly invoice to the company and payment at checkout. CHAOSRXOptics is part of CHAOS Supplies, a Phoenix, Arizona company founded more than thirty years ago and certified as a Women's Business Enterprise by WBENC.
This page is for safety managers, EHS leads and procurement staff who are evaluating prescription safety eyewear programs and want to compare the providers' ordering, fitting, billing and shipping options. It describes Eyelation only in terms its own website uses, and it does not rate either program.
What Eyelation is, in its own terms
Eyelation is a corporate prescription safety eyewear and safety footwear program provider with a benefits-managed platform. Its website describes ordering through an online marketplace and at on-site kiosks placed at participating company locations, with kiosk availability based on company size and volume. It also describes on-site services, including vision testing and product fitting events at company sites, and a benefits management platform where a company administrator chooses which products employees can see and sets the benefit as a dollar amount or a percentage. Eyewear orders call for a prescription upload and a profile photo. Frames are described as ANSI-rated. Orders ship to the employee's home or the company location.
Those descriptions are drawn from eyelation.com and may change. See the publication date at the bottom of this page. Eyelation is a trademark of its respective owner. CHAOS Supplies has no affiliation with Eyelation and is not endorsed by it.

Where the two differ most
Physical footprint. Eyelation offers online ordering as well as kiosks at participating locations. The CHAOSRXOptics portal requires no worksite hardware. Technicians at small shops and large plants use the same portal from a phone.
Fitting method. Virtual sizing helps employees compare supported frames on their own device. Any additional prescription measurements are coordinated directly.
Billing options. We invoice the company monthly on Net terms, or the employee pays at checkout, as the employer prefers. Mixed programs, where the company covers the base pair and the employee pays for upgrades, run on the same portal.
Scope. We do one thing. CHAOSRXOptics is prescription and non-prescription safety eyewear. We do not offer footwear, insoles or wellness services. A buyer who wants one vendor for several PPE categories should weigh that.

How a program starts
- Set the rules. You tell us the frame list, who is eligible, what the company covers, and whether billing is a monthly invoice or checkout.
- We build the portal. Your company gets a private ordering site with your approved frames and your ship-to options.
- Employees order. From a phone or computer, with virtual sizing for supported frames and prescription upload. No kiosk or on-site visit is required.
- We fill and ship. Glasses go to the employee's home or your office, shop or job site.
- It keeps running. New hires order when they start. Replacements order when they need to.
Claims scope
- Eyewear is imported.
- ANSI Z87.1 ratings are stated per frame on each product page. Do not assume a rating for a frame whose page does not state one.
- Nothing on this page is an OSHA approval claim. OSHA does not approve products. 29 CFR 1910.133 references ANSI Z87.1.
- No medical claims are made. A prescription from the employee's own eye care provider is required for prescription lenses.
- The Eyelation descriptions above come from eyelation.com. For current Eyelation program terms, consult eyelation.com.

FAQ

The CHAOSRXOptics program described here can operate through an employer portal without a required kiosk or on-site event.
Do employees need a kiosk or an on-site visit to order?
No. Employees order from a phone or computer on a portal built for the employer. No kiosk is installed and no on-site visit or fitting event is required.

Virtual sizing helps employees compare supported frames on their own device. Employees upload their prescription, and any additional prescription measurements are coordinated directly.

To the employee's home or to an office, shop or job site. The employer chooses the ship-to options when the portal is set up.
Do the frames meet ANSI Z87.1?
